Who is this patient, and what does that change?
Age, weight and renal function decide almost every dose that follows, so they are established before any drug is judged. The handbook's warning applies here: elderly patients nearly always have some renal impairment from ageing alone, and a normal serum creatinine can hide it when muscle mass is low.
